Job Description

Licensed Social Worker

JOB SUMMARY
This position is responsible for providing Social Work services to Home Health Care and/or Hospice clients and their families, as assigned.   The Social Worker is also responsible for assignment and working with volunteers to meet the needs of clients and families.  This position is responsible for delivery of quality service, promoting good public relations, and for upholding the mission and standards of the facility.

PHYSICAL REQUIREMENTS NEEDED TO PERFORM THIS JOB
This position requires traveling and physical stamina.  Ability to lift up to 10 pounds requires.  Travel required with the communities served.  Must have ability to be mobile within the assigned service area.

KNOWLEDGE, SKILLS AND ABILITIES
This position requires effective counseling skills.  Should be emotionally mature, objective and sensitive, and should possess a basic concern for people and their problems, especially relating to aged, disabled persons.  Must be able to handle responsibility, work independently, and maintain good working relationships with clients and team members.  Requires basic knowledge of health care funding sources, including Medicare, Medicaid and insurance.  Must be able to speak, read, write, and understand the English language.

WORKING CONDITIONS
This work requires emotional commitment and the ability to cope with clients’ emotional and stressful situations.  Works in clients’ places of residence, including apartments, tow houses, duplexes, and houses.  Travel is required from residence to residence and to community events, in all kinds of weather.  Must have reliable means of getting to each client’s residence on a timely basis.  May be exposed to pets and tobacco smoke in client’s residences.

Job Requirements

LICENSURE/CERTIFICATION
  • Must be certified by the State of Nebraska as a bachelors level social worker within two months of employment.
  • Valid Nebraska drivers license required.

 

EDUCATION/TRAINING, AND WORK EXPERIENCE
  • Graduate Masters from an accredited school of social work, preferred, or
  • Bachelors Degree, from an accredited school of social work, requires casework to be supervised by an MSW.
  • One year of clinical experience appropriate to the counseling and case work needs of the terminally and/or medically ill required.
  • Home Health Care, Nursing home or hospice experience preferred.
  • Training and experience in dealing with grief preferred.
